Central cord syndrome is an incomplete spinal cord injury. Commonly, it occurs in elderly patients with pre-existing cervical spondylosis or spinal canal stenosis who sustain a hyperextension injury.6 This type of injury is associated with bilateral sensory and motor function deficit, where upper extremity is weaker than lower extremity.6 At the meantime, lower extremity hyperreflexia and sacral sparing are noted.6 In this case, the patient is an elderly patient, with radiographically (x-ray and CT cervical) showed cervical spondylosis. He also presented with history of injury, even though the exact mechanism of injury was unsure. In addition, the patient presented with lower extremity weaker than upper extremity, hyperreflexia at upper extremities
