CCIB Intake received an email from resident Daniel Welty's daughter Debbie Welty, SMHC. According to the email Debbie is requesting follow up information regarding an investigation. The email stated Debbie disclosed that her father resident Daniel Welty had numerous falls at the facility during the months of October and November. According to the email the facility was responsible for keeping her father safe by providing one on one care and supervision in an effort impede serious injury. Debbie stated she is waiting for feedback or follow up from LPA Irma Valles and Brenda
