The homeless are a section of the population that has significant vulnerabilities and risks for poor health (Shi & Singh, 2015). The definition of homelessness is not standardized and varies by different government agencies but in general includes individuals or families without a regular private nighttime residence ("What is the official definition of homelessness?," 2016). The majority of the homeless live in cities; however, 1 in 4 of the homeless population are located in rural and suburban settings (Shi & Singh, 2015). The actual services to the homeless are provided by community health centers that are grantees of the HCH ("HCH fact sheet 2013," 2013). The homeless population can receive services at more than 2500 sites, administered by 243 grantees, distributed in all 50 states ("HCH fact sheet 2013," 2013). For example, Family Health Centers of Southwest Florida, Inc. is a grantee of the HCH in Lee County, Florida ("HCH directory," 2016). Family Health Centers of Southwest Florida Health Care for the Homeless Project provides healthcare services to the homeless population at three sites throughout the county and provides outreach services to the homeless at three additional sites ("Family Health Centers of Southwest Florida, Inc.," 2016). Primary care is available five days a week and includes evening hours ("Family Health Centers of Southwest Florida, Inc.," 2016). Care is provided for both acute and chronic diseases and is provided primarily by two Advance Registered Nurse Practitioners (ARNP) and the ARNPs are supported by an outreach worker and medical assistants ("Family Health Centers of Southwest Florida, Inc.," 2016). The Family Health Centers of Southwest Florida Health Care for the Homeless Project provides mental health and substance abuse treatment through partnerships with local providers ("Family Health Centers of Southwest Florida, Inc.," 2016).
