Client was recently discharged from Wyckoff Hospital on 9/1/2016. Client didn’t submit to this worker discharged paper. On 9/2/2016, CM met with the client to complete Bi-Weekly ILP Review and to follow up and hospital discharged. In the meeting client was dressed in red and black shirt set. Client was wearing a summer hat and reading glasses. Client appears to be cooperative and friendly. Client continues to suffer from lymphedema and open wound ulcer; which she was hospitalized at Wyckoff Hospital. Client didn’t provide copy of discharged paper. Client continues to display the following behavior. Client beliefs that "People are out to get client and how residents at the shelter are using Marijuana on both of her leg. Client
