Identification and Problem
The names in this case presentation have been changed to protect the identity of my clients. Tony is a 7-year-old boy who has a desire to be reunited with his biological father. Tony’s mother had her parental rights terminated 6 years ago, due to her non-compliance with her service plan agreement made by the Mississippi Department of Human Services (DHS). Tony’s father, Johnny, had his parental rights terminated in November 2015. On January 29, 2016, Tony was out to eat with his foster care mother, Theresa Kelly, and saw his biological father. The father immediately approached Tony and asked him if he knew who he was. Tony responded by nodding his head in agreement. Tony’s father then turned to Mrs. Kelly and introduced himself as Tony’s father, he then said his

Tony is to meet with a counselor, at the Sunnyside Behavioral Health Center once a week for the next 6 months. Tony will be addressing his issues concerning the absence of his biological father Johnny. Tony is also going to address his concerns about the house fire that happened. Tony is experiencing severe anxiety about going back into the home.
2. Tony is to try and get along with his classmates. Tony is to have a positive outlook on school and say something nice to at least two of his classmates. If someone does make Tony mad, he is to take 5 deep breaths to try and calm himself down. Tony has also been instructed to tell a teacher if another student is bothering him.
3. Tony is to treat Charles and Theresa Kelly with respect at all times. If something is bothering Tony, he has been instructed to talk with Charles and Theresa Kelly instead of getting mad and misbehaving. If he does get upset or frustrated at home, Tony should go to his room for a cooling off period and then talk with Charles and Theresa Kelly about what upset him.
