The Holland Code occupational interest inventory to determine Ms. Quarles' vocational interests will be included in the evaluation summary.

2) Ms. Quarles has some difficulties staying on task with assigned tasks as evidenced by her inability to remember what she was told to do. However, she is able to work independently without close supervision. She also is motivated to work as evidenced by her good attendance and punctuality. She also takes initiative to complete assigned tasks without being prompted or advised to do so.

3) During the review period of March 29, 2017 to April 11, 2017, Ms. Quarles has displayed behavior that is obvious that she does not like being told who to do. She also will not make changes when poor work or behavior
