On this date worker received a call from Ms. Angie Henderson, financial consultant at Walker Rehab. Ms. Henderson was calling to check up on the status of Ms. Lawrence's case. Mr. Lawrence passed away leaving Ms. Lawrence in need of a nursing home sponsor. Medicaid is requesting Ms. Lawrence have a nursing home sponsor. Currently Ms. Lawrence is having difficulties swallowing and is in need of a feeding tube. However, there is no family or friends willing to become the nursing home sponsor and Ms. Henderson is concerned and what the nursing home needs to do to get Ms. Lawrence a feed tube in place. Worker recommended checking with Ms. Ana Dyer, social worker at Walker Rehab and check on the two-doctor policy. This policy states that if two
