Focus: Skylar and her family will learn and utilize healthy communication skills to move towards reunification with her biological family. MS. Smalls (MHP), Skylar, Ms. Beaton (MHS) and Ms. Ross (Skylar DSS worker) debrief recent reports of possible self-harm and reunification.
Intervention: MHP, Skylar and MHS discuss reports from Skylar’s DSS worker of Skylar not communicating her emotions and not reporting conflicts to MHS. MHS encourage Skylar to speak freely during the visit. MHP request information about Skylar’s permanency plan. MHP encourage Skylar to express her feelings about all situations.
Response: Skylar express not having any problems, however did not deny reporting issues to her DSS worker. Skylar explain having problems with
