Case Study: Rigoberto

During the past 3 months Rigoberto did not have any hospitalization or ER visits. On 4/3/17 client was found with a swelling/bruise to his left eye; fracture to facial bones and left orbit was r/o with x-ray’s on the same day. He consulted Ophthalmology twice for the eye trauma; he was recommended oral Keflex that was not honored by PA/PCP but instead he received a Rocephin 1 gram IM once; Left eye trauma subsided w/o complications.
Noted an increased in seizure activity within this quarter. Client had a total of 7 documented seizures; he did not have any seizures the previous quarter. Rigoberto last Neurological consult on 2/8/17 with no new recommendations. Last change on antiepileptic meds was on 11/9/16 with an increase in Zonegran to
