1. The Appellant agrees to provide a completed PA 600R Renewal Application to the South CAO by close of business on March 24, 2016.
2. Upon receipt of this verification the CAO agrees to reassess the Appellant’s eligibility for SNAP benefits, by the close of business on March 29, 2016.
3. If the Appellant is eligible for SNAP benefits, the CAO will authorize those benefits and issue a new notice. If the Appellant is not eligible, the CAO will issue a new denial notice.
4. If the Appellant fails to provide the information, by the close of business on March 24, 2016, the original notice, dated February 8, 2016, will stand.
5. All actions will be completed by the close of business on March 18, 2016.
