Three Peer Transition

Through the last Month all three Peer Transition/ Diversion specialists have attended the five day Recovery Coach certification training and completed this successfully on 5/11, 5/12, 5/18, and 5/19. The Medical clearance for the three Peer Transition/ Diversion specialists has been completed for Catskill regional, but still waiting for the Justice center clearance to finally be a part of the Behavioral Health Unit. During this past month the three Peer Transition/ Diversion specialist has continued to co-conduct a discharge group, in collaboration with Community Service staff every Tuesday at the Behavioral Health Unit at Catskill Regional until to the final approval. During this last month we have continued to engage the previous two consumers. …show more content…
Mr. Decker appears to be utilizing his Wrap plan and remains hopeful about his future. Miss Armstead was served in the month of April and has carried over to this month to continue to help her in the Transition from Rockland Psychiatric center to the RSS group home on 2/10. The Peer Transition/ Diversion specialist continues to collaborate with the RSS Group home to work through some of the issues that Miss Armstead has been experiencing to support her success. Miss Armstead has developed an effective WRAP and Crisis plan, but needs assistance with adjusting and finding what supports her best. Miss Armstead continues to be connected to Mental Health services and her medical needs are being met. The Transition/ Diversion Specialist Continues to have weekly wellness calls with Miss Armstead and also face to face contact. Miss Armstead continues to sporadically do well when she follows her plan. Two individuals from the previous month are no longer being served consistently. Mr. Zeigler was originally referred through the Mobil Mental Health team. At this time Mr. Ziegler still seems to be weighing his options and not sure if he wants to work on a WRAP Plan and or connect to any of the available Mental Health services, we will continue to offer him hope for recovery. New referral through Community Services for Jail this month is Mark Seidenberg on 5/21, who was also seen on this day is in need of Mental Health services, developing a wrap plan and creating connections in the community, which Mr. Seidenberg appeared very eager to want to work on. Mr. Siedenberg does not have a release as of yet so, we will closely monitor. Three referrals this month came through Mobile Mental Health and are as follows: Alijah Toro on 5/21, Heather Avery on 5/08 and Julia Stephenson on 5/21. Mr. Toro is working on a WRAP Plan, being reconnected to Mental Health services and Medication monitoring. Mr. Toro arrived to us homeless, and in need of assistance with the Department of Family services. At this time Mr. Toro has been placed in the Motel and has Foodstamps. Heather Avery is working on a WRAP plan, housing and remaining abstinent from Alcohol use. Heather is connected to Substance and alcohol abuse
