Ms. Jones is a 53 year old female who presented to the ED following recently being released from Old Vineyard Behavioral health with concerns about medications. Ms. Jones stated she was hearing voices telling her to think about things per documentation. Dr. Osborne requested a consult with mental health. At the time of the assessment Ms. Jones s calm and cooperative. She reports recently leaving Old Vineyard and feelings her medications are not working. She informs this clinician she has an appointment with her outpatient provider, DayMark, tomorrow at 9:45am to meet with a psychiatrist. Ms. Jones denies current suicidal ideation , homicidal ideation, and symptoms of psychosis. She reports being at Old Vineyard for 30 days and while in there
