The reporting party (RP) stated approximately 3 weeks ago she visited with her relative, resident Harriet Kramer age 83 years. During the visit the resident was observed eating old, hard pizza. The RP described the resident as having dementia, osteoarthritis, and swelling of her arms. Conversely the resident was poorly groomed, had dirty hair, and smelled of dirty diapers. Subsequently she was sitting in a wheelchair with bruises on her wrist, hands, and feet. The RP stated the resident's son Craig Kramer is they POA/payee; however he resides out of state. His contact number is (425) 466-2175. It appears the RP spoke with the owner of the facility Patricia Smith regarding the care and treatment of her relative Harriet Kramer and was informed
