STATEMENT OF PROBLEM

T.O., a 10 year, 2 month old male was brought to the Florida Atlantic University-Communication Disorders Clinic by his mother, P.O for a Speech-Language evaluation. He was referred to the FAU-CDC by Dr. Goodell, T.O’s physician, for an evaluation due to a traumatic brain injury caused during a car accident 6 months ago. According to Dr. Goodell, T.O. is exhibiting difficulties in the areas of cognition, receptive language and articulation. It was also reported that T.O. has been withdrawn from social activities and his personality has changed since the brain injury.

BACKGROUND INFORMATION

Language:
The Peabody Picture Vocabulary Test, Forth Edition (PPVT-4), Form A, was administered to assess the clients receptive vocabulary and ability to respond to vocabulary instruction. The PPVT-4 consists of 228 test items with each set consisting of 19 sets, with 12 vocabulary words within each set. A basal and ceiling was determined to establish accurate testing measures. The client achieved the following scores:

T.O. received a Raw Score of 81, equating to a standard score of 56. This score falls within the .2% percentile, with a stanine of 1, and a Growth Scale Value of 128. These values equate to an age equivalency of 5:0 and a description of an extremely low score. The basal was established at age 6 with the ceiling being established at age 10. In total, T.O made 27 errors during the assessment out of a 60 opportunities. Every opportunity to identify a verb was missed. The remaining errors were distributed equally with no identifiable pattern observed.

The Test Of Problem Solving (TOPS), Elementary Revised was administered to assess the clients linguistic ability in describing a given situation, based upon a sequence of questions aimed to evoke answers. The TOPS was also used to determine the clients problem solving and critical thinking

Results of the Peabody Picture Vocabulary Test (PPVT-4) indicate an extremely low test score, with an age equivalency of 5 years old. His receptive language is significantly delayed and naming tasks appear to be difficult for T.O. Based on the Test Of Problem Solving (TOPS), test results indicate a cognition and problem solving impairment with an age equivalency of 7:1. T.O. has difficulty identifying social settings and recognizing daily activities. Results from the The Arizona Articulation Proficiency Scale, Third Revision (Arizona 3) indicates a mild articulatory impairment. Specific errors where made during attempts to pronounce the phonemes /θ/, /tʃ/, /r/ and vowels /aɚ/ and /ɛɚ/. The clients was observed to have intelligible speech despite having these articulation errors. T.O.’s attention did wavier frequently and redirection was needed several times throughout the evaluation. Based upon the results from the evaluations, focus of therapy should be on improving the clients receptive language and cognitive deficits. His prognosis is determined to be in question, due to his inability to consistently implement fine attention skills and the effects on his brain from the traumatic brain injury in the areas of cognition and receptive language.

RECOMMENDATIONS

It is recommended that T.O. receive treatment
