Mr. Dawkins was a passenger in the front seat of a motor vehicle that his friend was driving to take him to work. They were driving when a vehicle made a U-turn in front of the bar and hit them head-on. Mr. Dawkins recalls that the car was smoking and on fire and the airbag deployed. Mr. Dawkins recalls his face, head and hand were burned. His friend had difficulty opening the door and a bystander called EMS. The other vehicle fled the scene. They had to be extracted from the vehicle and the Jaws of Life had to be used. Mr. Dawkins and the driver were taken to St. John Medical Center on Moross in Detroit, while at the hospital he had his blood pressure taken in triage, he was told to sit back down. He has had diagnostic testing done including an MRI of his neck/shoulder/ back at Complete Imaging. He had placement of a percutaneous neurostimulator for his lumbar pain and felt that this didn’t help. He recalls having two injections into his left shoulder and that it helped a little bit. While receiving treatment at Mercyland Health Care, Pain Management, he has tried a P-stim that didn’t relieve his discomfort. He replied he was offered injections and surgery. He declined surgery on his back. He was evaluated by an Orthopedic Specialist, Dr. Paul Lewis and he replied he was told he could have surgery on his shoulder so he would like to proceed with this. Dr. Lewis felt Mr. Dawkins had a true rotator cuff tear and felt it was the result of the accident. Mr. Dawkins was provided with clearance paperwork for his PCP to complete and return. The paperwork remains pending and Mr. Dawkins claims he had the pre-op testing for clearance done at Harper hospital (EKG and pulmonary testing) and his PCP needs to sign
