Skylar made minimal progress towards this goal. Skylar was transferred to a new placement on January 20, 2017. MHS report Skylar is adjusting and requires monitoring in all settings for situations that leads to increase anxiety symptoms. Skylar report her anxiety level is increasing. Skylar will continue this goal.
