At this point of time, Officer Alonso contacted Dispatch and all the other Officers responded and advised dispatch that there would be an Off Property Escort with the patient. On 9/26/14 the Board received a complaint from attorney David Streett on behalf of his client Malik Karim against Dr. Stanley Librach of St. Louis. Karim’s complaint alleged that Dr. Librach had confronted him in his office in a threatening manner. Karim’s complaint stated that the Doctor’s actions gave him concern for his safety as well as the well-being of a business associate that was with him in his office at the time. Malik Karim notified St. Peters Police Department by calling 911 while the incident was in progress. Dr. Librach left the complainant’s office and property of 2730 South St. Peters Parkway in St. Peters while the complainant was on the phone with 911. On Saturday, 12/30/2017, at approximately 1024 hours I was dispatched as a cover unit for Officer Hyink, on the report of a husband who pushed their wife. IRTC observed documentation entered in Connections on 5/8/17 by CPS, Nichole Stuart, which depicts that CPS observed the subject child on 5/3/17. Subject child was observed with an injury to her ear which no one except subject child, Sophia, was able to provide an explanation; Sophia informed CPS that Xavier “fell and hit his face.” CPS is to assess the validity of Sophia statement and obtain any medical information regarding the injury. If it is deemed that it was inflicted by another person (adult or child) and it was a serious injury, CPS is to contact IRTC or an IRTC at ECS (after hours) for a case reassessment.…
CCIB received a complaint via phone call from RP stating on Friday 6/2/2017, she observed while visiting her daughter, child Oscar (last name unknown) (18 months) restrained in a type of highchair for 30 minutes or more. RP stated that Oscar has been observed to be in this chair multiple times, due to he hits and is too active for the teachers in the room (Ms. Veronica and two other teachers’ names unknown).
