At 8:05 PM Ms. Smalls (SAFY on call worker) receive an on call from Mrs. Clark (Foster Parent) that Skylar Wood was transported to Medical University of South Carolina (MUSC) for an evaluation. Mrs. Clark reported Skylar was redirected to complete her chores. Skylar became verbally aggressive, threats of jumping out the window, crying to go outside and refusing to comply with the rules of the home. Ms. Clark notice Skylar had new cuts on both forearms. Mrs. Clark state Skylar used her school identification to cut her forearms. Ms. Clark called 911 which the local law enforcement and emergency services responded. Ms. Clark explained the incident happened around 7:20 PM. Ms. Smalls notified Ms. Washington (SAFY Treatment Director) and the Berkeley
