Mr. Pena’s primary sources of income are public Assistance Medicare Medicaid and SSI Mr. Pena is currently receiving SSI in the amount of $633.00. Mr. Pena’s current mental diagnosis is Bipolar Disorder, and Anxiety. Mr. Pena reports history of psychiatric hospitalization as an adolescent, but does not recall the date, hospital and treatment provided. Mr. Pena also reports family history of mental illness.
Mr. Pena states he was sexually abuse by his cousin at the age of 7 year old. Mr. Pena reported no history of abuse or suicide. He also reports no criminal history, and stated he has a 26 year history of substance abuse: marijuana, heroin, cocaine, and alcohol.
