REPORT IS LATE DUE TO THIS BEING A REPORT FROM THE PREVIOUS SHIFT. EREPORT # 19420 STATED THE FOLLOWING: The incident happened on March 5, 2016. Ellen reported after she was admitted to Memorial Behavioral Health on February 5 and reported to staff that her father put his hands around her throat and choked her during a conflict in the home. Ellen reported this information to staff at Memorial Behavioral Health after she was admitted. Child reported that her father choked her and threated to kill her during a conflict at the family's home. Ellen has self-inflicted lacerations to both arms. There is a need for medical treatment. Ellen was referred to Memorial Behavioral Health for admission on March 5, 2016 after she cut her arms during a family
