Intervention/Response: CM traveled to consumer’s home for the purpose of monitoring and assessing needs. CM arrived at consumer home and consumer was sitting on the porch with his mother. CM met with consumer on the porch. Consumer had on mix match shoes. Consumer had a bruise on his head. CM asked consumer about his shoes and bruise. Consumer did not response. Consumer mother stated consumer bruised his head by running into the wall. Consumer mother stated consumer has been paranoid as evidenced by thinking spider was on his body. CM and consumer discussed consumer medication regimen. Consumer mother stated he has not been taking his medication. CM reviewed consumer pill planner. Consumer has not been taking his medication.
