D-The patient arrived late to her scheduled appointment and apologize to this writer. The patient reports that she is planning on leaving this clinic and will be going to the Hartford Dispensary on Monday to discuss the transfer. This writer discussed with the patient about the transfer process, at which the patient agreed to continue her treatment with CSAC until the transfer has been finalized. The patient reports she is stable at her current dose and haven't used any illicit drugs. The patient then signed her initial tx plan and this writer discussed with the patient as to what will be discussed during the next session pertaining to her tx plan goals, at which the patient agreed to the tx plan goals.
