1) Facility maintenance. It was annotated that water is seeping in around the office and storage room windows as evident of water strained walls and window sills. (29 CFR 1910.22(a)(2)) RAC 3=Moderate Recommendation: Submit work order request (DA Form 4283) to have the water seepage repaired and the walls and window sills painted with a moisture resistant paint.
2) Facility maintenance. Located throughout the office area, it was noted that the office walls approximately 12 inches above the base boards have some serious water damage. It appears that moisture was trapped between the wall surface and the wall, resulting in the wall surface to blister. Both offices (Manager and OPS) carpet is badly water strained, smelly and worn, due to the age of the carpet which can present a health risk. Old and worn carpet can harbor bacteria, mites and virus that can spread into the workspace. RAC 3=Moderate Recommendation: Submit a work order request (DA Form 4283) to have the carpet replaced.
4) Low illumination. Four (4) ceiling fluorescent lamps were burnt out. (29 CFR 1926.56) RAC 3=Moderate Recommendation: Submit a service order to have the burnt out lamps replaced.
5) Fire prevention. Located in both offices (Manager and OPS), the fire exit signs was not illuminating. (29 CFR 1910.37(b)(1)) RAC 3=Moderate Recommendation: Submit a service order to have the burnt out exit sign lamps replaced.
