Circumstance: Ms. Smalls (MHP) and Mrs. Cobbs (SAFY compliance Coordinator) made face to face contact with Ms. Cordova (caregiver) and Richard to complete monthly visit.
Action: MHP, Mrs. Cobbs and Ms. Cordova discuss the concerns of Richard having bed bugs. Richard did not have any paperwork ready to complete his monthly expenses. MHP and Ms. Cordova review monthly paperwork and medical documents needed for Richard file. MHP remind Richard to make a dental appointment. Ms. Cobbs check Richard’s room for personal property, door lock working and neatness. MHP schedule another appointment in the office for Richard to have list of monthly expenses ready. MHP wrote the check for room & broad and bus pass.
