Case Study: Las Casuelas Restaurant

On Wednesday, January 25, 2017, the Investigator traveled to Las Casuelas Restaurant, at 368 N. Palm Canyon Dr., Palm Springs, CA 92262 to conduct a scheduled employer-level AOE/COE investigation regarding claimant, Ms. Antonia Miramontes. At 9:00 a.m., three recorded interviews were received from the owner, Ms. Alana Coffin and two employees, Mr. Gerardo Martinez and Braian Ruvalcaba. The investigation revealed the claimant retained legal representation for her workers’ compensation claim and that Ms. Miramontes’ slip and fall was witnessed by Mr. Ruvalcaba that occurred in the restaurant kitchen. The incident was reported 24 hours later when Ms. Coffin spoke with Ms. Miramontes when she said she slipped and fell and was injured. She did not
