Case Study: In-Person Complaint

On Tuesday, January 26, 2016, the Department of Human (DHS) Office of Program Review, Monitoring and Investigation (OPRMI) received an in-person complaint/allegation. The complainant, Dalvaro Weaver (Mr. Weaver) arrived at 64 New York Ave, Washington, DC 20002, and reported that on Thursday, January 21, 2016, he had been sexually assaulted during his overnight stay at 801 East. He also alleged that several other incidents of “touching,” battery, theft, and sexual assault had taken place in various homeless shelters, hospitals, and at jails in DC, Maryland, Virginia, New York and Georgia.
Upon receipt of the complaint, Investigator Beekman (Investigator Beekman) conducted a face to face interview with Mr. Weaver. During the interview, Mr. Weaver provided a current District of Columbia (DC) photo identification and given the

Ms. Holt oversees the Homeless Services Monitoring Unit and assisted with obtaining information regarding Mr. Weaver’s stay at the shelter. According to Ms. Holt and Program Manager of Catholic Charities Archdiocese of Washington, 801 East Men’s Shelter, Willie Mobley (Mr. Mobley), there had not been any Unusual Incident Reports of sexual assault on January 21, 2016 at the shelter. It was also determined that data entry from the Homeless Management Information System (HMIS) indicate that Mr. Weaver’s last stay at 801 East had been on November 11, 2015. Moreover, sign-in sheets obtained from January 20, 2016 through January 22, 2016, indicate that Mr. Weaver had not been present on the date of the alleged incident. Mr. Weaver could not provide any specific details of the other mentioned allegations to include identification of dates, times, description of perpetrator(s), identify any witnesses. However stated he had made previous reports to seven Investigators and various Police Officers since
