On 4/3/10 at approximately 10:10AM resident Bettye walks into the laundry room in the facility, slipped on a recently mopped floor resulting in a break to her femur bone which leg unknown). Bettye was transported to Eisenhower Medical Center and had surgery to place a rod in her leg. Bettye remained in the hospital for a week until transported to The Fountains at the Carlotta Rehabilitation Facility in Palm Desert. The leg will take at least 6 weeks to heal.…

On Wednesday 06/29/2016 at approximately 0205 hours, Security Officer Christopher Paz (408) and Supervisor Steven Evans (407) were dispatched for a (51B) Disorderly Baker Act patient in E.D. room #27. Upon arrival, we went in the room and met with E.D. Charge Nurse Sharey Selover and the patient, Gene P. Glaser (DOB: 07/02/1963; FIN: #8533214). The patient had been brought in and Baker Acted by Police, he had been uncooperative, loud and wanting to leave. The patient, Mr. Glaser was spoken to by E.D. Charge Nurse Selover and it was explained to him that he had been Baker Acted and therefore was not allowed to leave. It was further explained to him, he must abide by the rules and procedures that are followed concerning Baker Act patient.…
The reporting party (RP) state on 7/8/16 at approximately 5:30PM a visit was made to the facility to assess resident Bessie Masich DOB: 12/16/17. The RP was notified by the Home Health Aide Abby who reported observing a bruise on the resident right breast. The RP disclosed the home health aide worked Monday, Wednesday, and Friday. Friday was the day the home health aide observed the bruise. The RP and the hospice agency nurse Carla Cordileone-Murphy examined the resident and observed several bruises to the right side of the resident's breast, chest, neck, shoulder, and under arm.…
On 10/25/17, at 1900 hours, in Durango 8 located at 3225 W. Gibson Ln Phoenix AZ 85009, Inmate Rubio, Daniel T391297 informed Officer Mossman B3777 that he was the victim of an alleged PREA violation while he was housed in Durango 9. Officer Mossman immediately removed Inmate Rubio from the housing unit for his safety and notified a supervisor. Inmate Rubio was taken to the Durango Medical Clinic by Officer Encinas B3226 where he was evaluated by RN Anthony 1247H. Inmate Rubio was cleared to be re housed at 1922 hours. According to the caller on 1/26/17 resident David Wood met with his physician Dr. Scott (951) 443-2200 for an examination and an assessment. During the examination David received blood test which tested negative for illicit drugs. Subsequently David's physician increased his psychotropic medications; however found he did not meet the criteria for hospitalization. On Saturday, May 12, 2018, at approximately 12:31PM, I, Officer Larry Thomas was dispatched to the New Patient Tower, (room E.421), in reference to a patient control. Upon arrival, I observed a black male subject who was identified as Durell Childers walking around the room talking to himself looking around. I asked, Mr. Mac Mcneil the patient nurse what happened. Mr. Mcneil stated that the patient took the picture frame down from the wall that was inside his room, so he went inside the room to see what was wrong. I am Shakira Nelson, a social work intern within the Substance Abuse Treatment Program (SATP) at the Fayetteville Veterans Affairs Medical Center. The Substance Abuse Treatment Program provides intensive outpatient treatment options to Veterans, and within the program the staff includes four licensed clinical social workers, three of whom are licensed clinical addiction specialist, program coordinator, psychiatrist, and nurse. The Veteran is a 69-year-old Vietnam Era Veteran who is 90% service connected related to the medical conditions Arteriosclerosis, Diabetes Mellitus, an eye. He was seen in SATP during walk-in hours, which are 0830-1500.…
Elder Mistreatment Case Study This case study was concerning the health and safety of Mr. Edward McKay in his home. Mr. McKay is an 82 year old gentleman who after suffering two cerebral vascular accidents has been left with left sided hemiplegia and is unable to speak. He is bedridden with bilateral lower extremity contractures, incontinent, dysphasic and dysphagic. Mr. McKay’s physical condition alone would require total care on a 24 hour basis to assist with feeding, changing and turning him. He lives with his only daughter and son in law and has no other people who check on him or assist with care in his family.…
Residents placed in a long term care facility is a hardship alone on the resident and families. Some families are in need of a little more assistance with their family member. This help may be required for many of the activities of daily living or needs that healthy, active people take for granted and may include such things as walking, bathing, dressing, and using the bathroom. Being a consultant and reviewing the long term care survey which receive several deficiencies notation for noncompliance with federal requirements. Much of my work during my internship thus far has been behind the scenes, mostly finding suitable placements for patients on the active discharge list. Therefore, my one on one interactions with patients has been minimal at best. However, thinking back to my first month at Western State Hospital (WSH), I remember a patient who was having trouble with being discharged.
