SC received call from Shreya Hill, director of nursing and Pa’s scheduler/ visiting nurse at Moravia Health Network on 10/08/2015 call lasted 4:12-4:20 PM. Shreya stated that she spoke with DR. Igway and wanted to touch base with SC. SC explained to Shreya the rationale behind the desires for closer supervision. Shreya stated though she understand it would be difficult for her to do on frequent bases as she only see’s Pa every three months. Additionally, she stated that at her last home visit in August, 2015 PAS was not present. But she has no reasons to doubt PAS is carrying out her duties. Shreya states another family member (Pa’s sister PAS mother) receives PAS service from the agency and sometimes she sees them altogether at Pa’s sister
