Case Study: Donovan Orienting To The Home

Circumstance: Ms. Smalls (MHP) made face to face contact with Ms. Canady (MHS) and Donovan for weekly visit.
Action: MHP and MHS discuss Donovan orienting to the home. MHS and MHP review paperwork and needed services for the youth. MHP interacted with Donovan on the floor with blocks. Donovan was not talking however he did make sounds. MHS explain Donovan does talk a little in the home. MHS expresses concerns that Donovan had little clothing and refuse to sleep in the room alone. MHP inform MHS of the youth’s new DSS worker. MHP review MHS questions and emailed the new DSS worker (Ms. Holmes). MHS questioned Donovan’s level of functioning. MHP explain Donovan needs assistance with everything including cleaning himself.
Plan: MHP will continue
