Circumstance: Liam will be healthy and free form severe impairments from the diagnosed DisGeorge Syndrome. Ms. Smalls (MHP) made contact with Wigfall (MHS) discuss Liam review medical paperwork.
Action: MHP and MHS discuss Liam’s medical update and any changes with medication from recent medical appointments. MHS reported Liam had no reported illness associated with his medical disorder. MHP observe Liam running around the home, picking up toys and following MHS around the home. MHS expresses Liam is very active around the home and gets up early in the morning. MHS provide information about upcoming medical and therapy appointments. MHP and MHS complete SAFY monthly paperwork. MHP and MHS schedule face to face appointments for the month of
