On Saturday, August, 20th, 2016 at approximately 02:00 PM, resident Ung Gi Koh went out to the Reed front lawn to meet the Erie Metropolitan Transit Authority Administration Bus that resident Koh thought he left his keys and ID card on. On Wednesday at 12:03 AM Resident Director Brandon Winslow received a phone call from Resident Assistant Christian San Pedro regarding a resident of concern. Resident Maryssa Hodder and Resident Marena Horan came to Resident Assistant Kayli Boyes with concerns on the whereabouts of Resident Falon Nonemacher who also resides in McHale 327. Resident Nonemacher was not answering phone calls from both Resident Hodder and Horan. RD Winslow was informed that Resident Nonemacher left to go home on Tuesday October 3rd around 1:00AM. Writer and BHCC Care Coordinator Nancy Gaskin had a Telephone conference with Paul Green (RCM) and Angela (Assistant Supportive Housing Director). RT was also present at the time of the call in the conference room, but was quiet and did not appear to be coherent. Writer and CC both explain to Paul Green and Angela of RT's behavior. CM Paul Green informed writer and CC that RT has not been taking his medication and appeared to just be worried about receiving his check and not his well-being. The incident occurred on 9/1/16 when she stated she was hit in her room hard by a staff with a fist. In the report, the RP stated on 2/28/2017 she received a phone call from Fatuma Nassanga (designated administrator), stating that the facility had ants and bedbugs. RP stated that she instructed Fatuma on what to do for the bed bugs and to help the residents. RP reported she instructed Fatuma to have an exterminator spray the facility, wash all the residents’ clothes and belongings, also to have all the residents cleared by their physicians and SIR’s for the residents by 2/28/2017.
