Ayden is up to date on all medical appointments. The youth physical health and disabilities is stable with his current medical conditions. He does not have any known food or environmental allergies. Ayden’s is maintaining a stable weight, however MHS is concerned with the weight gain with Ayden not being physically active. Ayden is alert and display more physical movements through his weekly PT and OT. Ayden continues to make minimal gains from therapy services. Because of frequent medical appointments, the youth does not get sick often.
8. Ayden does not have age appropriate behaviors. The youth functions as a 24-month old child. Because of Cerebral Palsy and other medical conditions, the youth lacks age appropriate developmental, emotional and behavioral functioning. The youth receives weekly PT and OT.
9. Per referring agency, Ayden has a bi-weekly visitation plan. Ayden’s family visits are scheduled by the referring agency. Ayden did not have any visits during the past 30 days.
