The counselor met with Loc for his Addiction Index Severity assessment. Mr. Tieu arrived on time with his brother for his scheduled appointment. Mr. Tieu is a 33 year old Vietnamese male recently divorce 2 months ago. Mr. Tieu currently lives with his parents, brother, sister-law and niece. He reports being released from HCPC hospital on August 25, 2015 where he stayed for a weeks. Mr. Loc reported the last grade he completed was the 10th. He, stated he, dropped to join a gang called the LT. He also report that synthetic marijuana is his drug of choice and prior to admission into the hospital he purchase $12.00 worth of KUSH and roll 5 joint. Brown for our schedule individual session. Mr. Brown reports that he is slowly decreasing He reported drinking one 40 ounces Old English this week. He also reports being sober for 7 days and want to rewarded himself with the beer. Mr. Brown denies any HI/SI at the time of session. He also has attended 6 meeting at Third Tradition AA meeting. The counselor praise the client for being able to stay sober for six days in a row. Mr. Brown appears to be in the contemplation stage of change as evidence of willing to seek residential treatment for his substance abuse problem. The counselor next scheduled individual session with Mr. Brown is on Monday August 31, 2015 at 1130 am.

The patient and I discuss him building a sober support system to help with him maintain his sobriety. The patient said he knew where an AA meeting was in his community the counselor suggest he try that one first. The client is to attend two meeting before our next individual session.
