I am writing to request home visits for Mrs. Harward, a 73- year- old widow, who needs assistance after being discharge. She was admitted to the hospital on 30/6/2012 following a fall while descending stairs.
She was initially presented with pain and weakness in right shoulder and was diagnosed as a right partial rotator cuff tear.Therefore, surgical treatment was suggested. However, she preferred non-surgical treatment.
She is prescribed with tablet ibuprofen 500 mg four times a day.She has been suffering from diabetes mellitus type 2, for which is taking tablet Metformin 500 mg. Following her discharge,Please monitor her blood glucose level every four hour which may be elevated due to administration of cortisone.
Socially, she lives independently.
