Case management is a challenging practice that involves establishing and providing health and human services to clients in need by carrying out the similar steps of the nursing process. The goal of case management is to actively promote quality and cost-effective interventions in order to receive optimal patient outcomes. In our book, case management is defined in public health nursing as the ability to optimize self-care capabilities of individuals and families and the capacity of systems and communities to coordinate and provide services (Stanhope & Lancaster, 2014, p.233). The case manager plays many vital roles in the case management process in order for it to be successful. Nurses often times carry out the many roles of the case manager because the care of the client by the nurse involves continuation of care out in the community. It is important for the nurse that seeks to take on the functions of case management to understand the roles that the case manager entails, in order to provide patients with quality care. Case management is a blooming field that is individualized and requires continuing of education via certifications in order to show competency in the area of case management.

The case manager whether a nurse or not, takes on many roles that makes the case management process able to achieve the goal of patient outcomes. Continuing education and certifications are important goals for case managers themselves that can result in high quality, cost effective care. Case management is ultimately the bond that connects hospitalized patients with community health care.
