NSG 4998 Objective 12: The professional nurse utilizes principles of case management in the coordination of health care of individuals across the lifespan.
Describe your use of case management principles in coordination of healthcare of individuals across the lifespan. The basic concept of case management involves the timely coordination of quality services to address patient specific needs in a cost-effective manner in order to promote positive outcomes (Case Management Society of America, 2010). As a House Supervisor working in a rural healthcare system, I have learned that each position within the healthcare team serves as a case manager. Each morning the healthcare team comes together and reviews each patient’s needs. From this meeting each patient will know exactly how many days he/she will stay and the expectations for the duration of his/her hospitalization. The nurse will provide wound care, assist with activity orders, and pain relief management in the form of medications, hot/cold packs, and other various modalities. This will prepare the patient for rehabilitation throughout the day and set the patient up for the best environment to …show more content…
The nursing license that I have earned from the state of Wisconsin proves that I have the basis of knowledge and understanding in regards to health and illness along with an understanding of age appropriate, culturally meaningful educational information. My job description shows what is required of me through my qualifications and skills needed for daily job functions. It also provides insight to the essential skills I perform in the House Supervisor role. My professional certifications allow me to further my knowledge in specialty areas and participate within other departments in the hospital systems as needed. The College of St. Scholastica course syllabuses demonstrate my commitment to becoming a holistic nurse who is a leader in improving nursing practices and continuing education. The state of Wisconsin does not require Continuing Education Units (CEU’s), although we do quarterly Computer Based Training (CBT’s). This training is modified throughout the year and CBT’s will be added or deleted as new evidence based practices change. I also complete Age Specific monthly tests (6 tests required yearly) as a requirement for my hospital organizations nursing education. My annual evaluation provides evidence of my work ethic and
