D:Client stated that she was referred to come to the SC from her brother-in-law NC# and she was interested in case management. Client stated that she was sleeping in a abandon building before coming to Housing Forward. Client also said that AS# is her sister and the last time they saw one another they had a fight. Client stated that she was interested in finding a job as soon as possible. Client has not been working since last year.

A:I observed that the client kept on scratching her wrist and she could not see the intake form really well, so client may need a eye appointment.

P:Make an appointment with case manager and have client complete the career passport application for the employment readiness program.
