Introduction: In 1951, Miller HM et al, first reported dental autotransplantation or autogenous tooth transplantation (1). Autogenous Tooth Transplant is defined as the transplantation of teeth from one site to another in the same person (2). The recipient site is either immediate extracted tooth socket or a surgically prepared alveolus. When there is an available donor tooth, ATT is a viable treatment option for replacement of a traumatized tooth (3). According to Mejare B et al, Molar tooth loss may have different etiologies (ie) grossly decayed, failed root canal treatment, unaffordable patients and tooth fracture/ perforation (4). An appropriate case selection and consideration of all biological aspects leads to a better out come in ATT. The preservation of PDl is necessary for survival of the transplanted tooth and may result with ankylosis or rejection (8).
Aim: The aim of this case report is to demonstrate a successful replacement of grossly decayed untreatable second mandibular molar tooth with its adjacent completely developed third mandibular molar.
Case Report: A 22-year-old female had reported to our dental department with a complaint of pain in relation to her right lower posterior teeth since 1 month and had been under medication since then. Clinical examination revealed good oral hygiene but the second molar was grossly decayed. Periodontal probing depths did not exceed 3 mm; however, the involved tooth was tender to percussion and palpation. The radiographic examination showed caries in the second molar. The patient was informed about the decayed tooth; and extraction was recommended to her. The examinations also showed that adjacent third molar was healthy,favorably developed, completely erupted with a slight gum coverage along the distal aspect of the
