Carousel Pharmacist Interventions

Decent Essays
In this week of rotations, I shadowed the pharmacists and pharmacy technicians at Comer’s Children Hospital Pharmacy at UCM. I learned about what the carousel is and got an opportunity to use the carousel in the pharmacy. I helped the technicians fill oral solution syringes for pediatric patients. I also shadowed the pharmacists and learned how to verify orders on Epic Systems, and worked on some pharmacist interventions. This visit helped me get a better view of the important roles of an institutional pharmacist. When I got the opportunity to see the pharmacist work on the interventions I learned that this is very important for hospital pharmacists because the pharmacists’ role is to prevent medication-related problems (MRP). In the Bachelor of Nursing curriculum, the 7 rights and 3 checks of medication administration is taught, practiced and performed by all nursing students to ensure proper habits are developed. These short cuts, although time saving, ultimately cause more complications for the patient and the health care system. As a group, we have found that CARNA’s Medication Administration Guideline (2016) is not being implemented or enforced as it should to prevent medication errors.…

The key stakeholders involved in this project are the patients, the pharmacy team members (Pharmacists, Technicians), other Healthcare professionals (physicians, nurses), the independent pharmacies that are currently contracted to service the LTCs, and the Health Region Executive members. The patients will benefit from the business plan as they will have increased access to the Pharmacist’s clinical expertise, leading to a clearer idea of medication regimen upon discharge and better outcomes. Nonetheless, they may be concerned about the automatization of medication dispensing and the potential for errors. They can be re-assured that the technology is not error-prone, and that there are checkpoints to assure the correct patient receives the…
