I am a Social Service Case Manager at Caritas Family Solutions in East Alton. I work closely with Christopher Duncan (DOB 03/19/1983) within his involvement with Caritas Family Solutions. Christopher has an open case through our agency. As part of Christopher’s case I work alongside Christopher to refer him to outsourcing services.
I am writing this letter to verify Christopher Duncan’s identity. Therefore, he may be able to receive valid identification to engage in services Christopher needs to be compliant with Caritas Family Solutions agency.
