Client is a forty two Caucasian male that has an extensive history of substance abuse that has effected all every area of his life. (Legal) Client has a CPS case pending and will be going to court for custody of his children pending the outcome of treatment program completion and his C.P.S requirements (Family and social relationship history) Client reported that his girlfriend is currently in a treatment program at West Care. The client reported having low self - esteem client appeared to be open and honest in his response to the question asked on the ASI, and seems to want recovery for the most part It is the assessment that the client may have been embarrassed about having his kids taking because of his addiction.
