Goal: CM met with client for weekly case management. I: CM inquired about client’s health and skin condition. CM reintegrated program rules and reminded client that no drugs/paraphernalia are allowed on the program's property. CM agreed to help client file a new claim by providing a referral to the Los Angeles Vet Center.CM discussed housing options with client, and confirmed client’s desire to attend VASH screening clinic in the upcoming month. CM also assessed client’s motivation level, by using open-ended questions. CM continued to assess for PTSD symptoms, substance abuse, and medication compliance. R: Client presented well groomed, while his skin condition had improved from the previous week. His affect was appropriate and consistent
