A study demonstrates 25% patients hospitalized with heart failure are readmitted with in 30days. The Self Care College was implemented in 2011, the hospital has been reduced readmission rate from 16.8% to 12.85%. The hospital implemented simulation education to ensure CHF patients are able to manage their health. The learners will participate in 3 different stations weight, diet and medication. For each they station, the facilitators asked them to perform the skills they will have to do to at home. If learners are incapable to perform the skills, the facilitator will go over and show them the correct way to perform it. After the learner are properly trained they are qualify to be discharged. According to this article simulation education for hemodialysis patient give them the ability to cope with dialyzing independently at home. The dialysis staffs have been using simulation to practice their skills. Since simulation is one way to learn and practice in a safe environment, they decided to create a program that can help patient to perform self-dialysis at home. The innovation room program was implemented from July 31th 2011 to August 31th 2014. It is a training room that is equipped with bed, desk, television, toilet, dialysis machine, portable reverse osmosis machine as well as supplies required for self-cannulation. The only communication permitted between the training patient and the primary training nurse was the phone. Depending on a patients performance in the innovation room the primary nurse determines a patient’s readiness for home. At the end of the program to patient is able to perform self-care at home which is ability to cope with dialyzing independently at home. This study has shown that simulation –based teaching in patients training for home HD may provide an important supplement to traditional teaching strategies in preparing patients as they transition to the home
