Attempt 1= CFT/initial meeting
CM attempted to conduct an initial meeting for Omar (youth) today, 6/20/17 at the family home at 5:30pm. In attendance were Jasmine Alexander (CM), Ivan Flores (CM), Ingrid Watson (FSO partner) and Omayra Centeno (caregiver). Caregiver contacted youth during initial meeting via phone regarding youth whereabouts. Youth reported he will be home in 5 minutes and did not attend initial meeting after 45 minutes. CM explained to caregiver about CMO, CM’s role and services available to youth and family. Caregiver stated that she is interested in working with the CMO. CM explained to caregiver that CMO services is voluntarily and CM explained Hudson Partnership no contact policy. CM noted youth has to be present for
