D/A: On 01/16/17, Mr. Brown was supported to his appointment with Dr. Ramani and on 01/18/17, went for CAT Scan at 9: 00 a.m. Mr. Brown continues to attend dialysis three times per week. On 01/21/17, Mr. Brown was on his way for dialysis treatment, when he suddenly started complaining of severe chest pain. The bus driver had to divert and took him immediately to the Kent General Hospital, where he received his dialysis treatment and treated for chest pain. On 01/19/17, Mr. Brown went for dialysis at 6 a.m., because Fresenius Medical Center called to change the time and asked for consumer to take his morning meds before going for his treatment and also to bring sandwich, yogurt and small water bottle of water for lunch. On 01/16/17, consumer refused his dinner, stating that the dialysis treatment “makes him feel that way.” The consumer was advised on several occasions about walking shirtless around the house but ignores staff. On 01/16/17 and 01/17/17, Mr. Brown was incontinent, he was assisted with shower. Staff found a new pack of cigarette in his pocket. Mr. Brown was prompted to take shower on 01/17/17, 01/18/17 and 01/19/17, to which he complied. The consumer refused his dinner on 01/17/17 and also on 01/19/17 refused to eat his breakfast. Staff noted in the communication log that on 01/18/17, the consumer refused to take his 9 a.m. medication and also refused to get up from bed to sign refusal form. On 01/18/17, Mr. Brown called the house phone with his cell phone while in bed asking if “someone can put Nicholas on the phone,” he will like to speak to him. Mr. Brown was redirected and told to come to the living room if he so wish to speak to Nicolas. On 01/18/17, staff made several attempt for Mr. Brown to come take his medication but refused until staff finally went into his room. The consumer became very upset, but complied to take his
