Presenting Problem:
Pt is a13 y/o white male presented to NNBHC accompanied with his mother due to expressing SI to his school counselor today. Pt has hx dx of Anxiety D/O and Depressive D/O. Pt states he is currently SI with a plan to jump of his bedroom window head first. “ I wrote a letter to my best friend yesterday I want to die”, “Im fat and ugly”. Pt states of self harming behaviors such as punching his leg. Pt states that he has been a victim of bullying since the 7th grade by the same individual. Pt stated “He said he was going to murder me yesterday”. Pt endorses HI with a plan, “I’m going to bring a knife to school and slice his harm to make him feel how I feel”. Pt denies psychosis or paranoid thinking.
