Building Relationships With Client In The SNF: Case Study

Coaches are currently following-up with their clients in the SNF. Would you like this process to continue or are we referring all SNF clients to Elizabeth? I have asked coaches to reach out to Elizabeth for follow-up once their clients are in the SNF. Elizabeth is building relationships with the SNF. I would prefer that Elizabeth assume this role. It allows for more consistency across the process. The social workers will know Elizabeth. No introductions will need to be made with each coach coming into the facility.

I want to send out an email to explain the process but want to make sure this is the process you want in place.
