Sharonda Bryant fulfills her expectations for this rating period. Sharonda is the REV Team SOSA, which is a new clerical position in our region. Sharonda has been with the agency, as an Hourly and Intermittent employee for several years. Sharonda has learned much about the policies and procedures of the agency. Sharonda is knowledgeable of all of the web based systems used to complete her tasks. Sharonda is also familiar with how to process care plan requests if necessary. Sharonda’s experience with this agency has given her the background to excel in her current position. Sharonda is knowledgeable in processing incoming provider communication faxes, answering phone calls in Cisco Finesse Call Center system and documenting in FRED and Web Tool. Sharonda also creates FRED team assignments for Reassessment/Service extensions upon receipt. Sharonda files paperwork according to her assigned alphabet coverage. There have been no problems reported.

Sharonda performs all tasks placed before her as requested. Sharonda asks questions for clarification if she needs more information. Because of Sharonda’s background with the agency, she is able to screen out requests to determine whether or not they are REV team requests or if they need to go to the Assessor Team for face to face visits.

Sharonda submits leave requests ahead of time and ensures that her leave requests are prior approved by her supervisor. Sharonda enters her daily time and leave use in DELTA and submits her timesheets in a timely manner. Sharonda also updates her calendar in Outlook when leave has been approved. Sharonda has maintained appropriate leave balances to avoid leave without pay this rating period. Sharonda complies with Departmental policies and procedures. Sharonda has done a good job in prioritizing her assigned duties and ensuring that her tasks are completed accurately and in a timely
