Flexeril 10 mg tablet- 1 Tablet, 3 times per Day, trazodone 100 mg tablet- 2 Tablets, 1 time per Day, PRN take 1 to 2 tabs for difficulty sleeping, elavil 25 mg TID
Patient is a current resident at the Sober Living of America and unable to fill any of these medications. discussed with the patient add Vistaril 25 mg capsule- 1 Tablet, 3 times per Day, as needed for anxiety, out elavil 25 mg, which is a $ 4.00 medicatin at the warmar, and patient stated she cannot even afford the $4.00.
