Mr.___ ___ [NAME]returned to the uncology clinic for follow up of stage 3 bladder cancer under line. Radiation to the bladder and that was completed 2 months ago. He feels well at this time and he has no complaints. He denies any pain. no urinary problems, no coughing or shortness of breath.
On Exam he weights 186 pounds, blood pressure 133/60, pulse 57, temperature 96.9. No oral flairgel lesions. Neck supple, symmetric and lungs are clear heart was regular rate and rhythm.
