Birth Center Delivery: A Case Study

Belinda is a 24yo, G5 P3012, who is currently 23 weeks 2 days as dated by LMP. She has not any PNC as she recently moved to Tennessee. Obtaining insurance was quite difficult and she just now came into care. She has a history of chronic back pain but does not take any medication. She has 3 prior full-term deliveries. She does remark that she has had some elevated BP’s during pregnancy previously but has not had any complications to date. Her BP is normal today at 125/84. She is planning a delivery at Lisa Ross Birthing Center.

On ultrasound there is a live fetus in cephalic presentation. Fetal biometry is fairly symmetric and 6 weeks ahead of menstrual dating. A detailed anatomic survey was unremarkable. There were no structural abnormalities noted within the capabilities of our ultrasound equipment, although the profile was somewhat limited. There were no common markers of aneuploidy seen. Fluid and Doppler’s are reassuring and BPP is 8/8.

We redated her today giving her a new due date of 01/26/17. We discussed her plans to deliver at Lisa Ross Birthing Center. Even though she is 29 weeks I do feel she is a candidate for a birth center delivery. I asked that she call and make an appointment either today or Monday as she will need a Glucola now that she is slightly further along. We scheduled her to return in 3 weeks to confirm dates as well as complete the anatomic survey if
